Development of the All-on-4 procedure

The All-on-4 treatment protocol was developed in the 1990s by Nobel Biocare, a world leader in innovative dental implant solutions, in conjunction with Portuguese dentist Dr. Paolo Malo. Nobel and Dr. Malo were dedicated to finding a system that could effectively provide implant-supported, full-arch replacement teeth to patients who had lost a lot of bone in their jaw and to be able to do so without having to rely on bone grafting.

They developed a special tapered implant that had top-to-bottom threads that would optimally integrate with the jawbone. They then found that placing the rear implants at a slanted angle would take the best advantage of the available bone in the jaw and eliminate the need for bone grafting in most patients.

Nobel and Dr. Malo also found that four implants strategically placed in the densest areas of the jawbone gave the strongest support. Another major breakthrough came when they applied the mechanics of bridge construction to the creation of the new teeth that go on top of the implants.

The All-on-4 procedure uses a single set of teeth solidly fused together so that the forces generated by biting and chewing can be evenly distributed. This is why patients receiving All-on-4 treatment can immediately use their new teeth after the procedure.

Dr. Rapoport and Dr. Schuler can place special, extra-long implants for patients who have experienced severe bone loss in their upper jaw. Known as zygomatic implants, these are four to five times the length of conventional implants and are placed through the jawbone into the cheekbone.
